This form is required to release a video or audio NFT on OpenChord.

Be prepared to submit important information about you and your video to ensure a smooth video or audio release experience.

First name, last name, email address , and wallet ID (Public Eth Address). If you don’t have a wallet it’s easy to obtain.   We recommend  Metamask.  It’s the most popular.

 You will need to check your email for a code to verify your email address.  Input the code and you will receive an email with your login information.
